Support
Services
acts as a “unifier” in the mental health care system by linking
young people and adults to services specific to their needs.
Support is tailor-made, matching community programs to each
individual. By integrating and personalizing services, care
is efficient and effective and provides a solid building block
for self-reliance and self-development.
Adult Services ~ Adult Case Managers provide the
bridge between the person and mental health support systems
for adults. The Case Managers monitor service delivery and respond
to changes in a person’s needs. Direct care is also provided
through teaching, community integration, supportive counseling
and crisis intervention.
Child
& Adolescent Services ~ Youth Case Managers help children,
adolescents and families identify and benefit from services
at schools, clinics, health care centers or social services.
They offer support by providing short-term and informal counseling,
referral, and transportation.
Benefits Management ~ This service assists with money management
and benefits. Representative payee services are available to
those receiving Social Security. Individuals are also assisted
in applying for benefits and in social security appeals, hearings,
re-determinations and reporting changes.
Supported
Housing ~ Supported Housing helps those ready to leave psychiatric
centers or community residence programs, as well as homeless
people with psychiatric disorders, in finding quality, permanent
and independent housing. A person is assisted in locating and
moving into a new home: finding a location, evaluating a lease,
selecting furniture. After having settled into a new home, clients
regularly work with the AMH Housing Coordinator to maintain
stable living in the community.
Dual Recovery ~ Dual Recovery Case Managers provide systems
integration by improving client access to comprehensive services
and continuity of care, facilitate sharing of information, and
collaboration among our agencies to improve outcomes for the
homeless, and provide a linkage with the mental health and substance
abuse services, housing, health-care and income supports and
entitlements. The target population includes persons 18 years
of age and older who are homeless and have a serious mental
illness with an emphasis on those who have alcohol and/or drug
problems and are involved in the justice system.

Caleo Counseling Services, the outpatient clinic of AMH, strives to provide counseling and therapy in a warm and professional atmosphere. Services are available for children, adolescents, adults and families living in Warren and Washington Counties.
Group Therapy ~ Peer interaction and support can be vital to treatment and recovery. Through group counseling, the client gives and receives feedback with others who have direct knowledge and experience of his or her problems. Groups are facilitated and guided by a staff therapist with specialized training in the particular group area.
Sexual Offender Program ~ Caleo Counseling Services, in conjunction with St. Anne Institute, has developed a treatment program for sexual offenders. Directed by a Caleo or St. Anne psychotherapist specializing in sexual abuse and offense, the program offers comprehensive assessment, evaluation and therapy for children, adolescents and adults. Individuals enter the program through self-referrals or community agencies throughout Warren and Washington Counties (e.g. family courts, probation and parole, legal departments).
Treatment Planning ~ Treatment planning begins with a concentration on community safety issues and problems specific to the individual. The course of treatment is developed in collaboration with the client, the referral source and Caleo/ St. Anne therapist. The plan and progress are reviewed periodically.
Treatment ~ Treatment is based on the needs of the individual. We offer individual or group psychotherapy as well as couples and family treatment. These approaches are not exclusive and may be used in conjunction with each other. A Relapse Prevention model is the primary treatment approach.
Eligibility ~ While the majority of people served reside in Warren and Washington Counties, we do offer services to people outside of this area.
In-Take ~ To begin the in-take process, call Caleo Counseling Services, 9am - 5pm, Monday through Friday, 518/747-8243.
Residential Programs
AMH
residential programs are houses and apartments where individuals
with psychiatric disabilities live while developing skills needed
for independence. Programs are designed as tiers, each offering
a different level of support in a person’s effort to achieve
his/her highest level of well-being. Services promote a person’s
capabilities through companionship, education, counseling, vocational
training and respite; they reduce costly hospitalizations by
providing caring and skilled intervention in times of crisis.
Available to those 18 or older, the duration of stay is based
on individual need.
Residences
~
Genesis & Pearl Street Genesis and Pearl Street are the
first tier of the residential system and offer a group home
environment for people in the earliest stages of recovery. The
overall goal is to provide short-term, focused skill development
in a home-like setting.
The first step in treatment is defining and developing a rehabilitation
plan which aids a person’s movement to the next tier. This individualized,
highly detailed plan clarifies needs, charts a course of action
and provides the framework to evaluate progress. Services available
to residents include crisis intervention, rehabilitation counseling,
social and living skills development (money management, personal
hygiene, food preparation, meal planning, recreation activities),
symptom and medication management, community integration and
resource development.
On duty around the clock, staff work closely with each resident
to maximize stability and help in the transition to a less restrictive
environment. Longer term assistance is available for people
with problems preventing them from living outside a group setting.
Community Living Apartments-
Maple Street & Satellite Apartments
The Community Living Apartments Program offers two types
of living arrangements: Maple Street, the second tier of the
residence program, and the third-tier Satellite Apartments.
Both include support and guidance through individualized service
plans created in partnership with the service recipient.
Services are a continuation and expansion of those in the first
tier, and carry the ultimate goal of achieving an independent,
successful life. Residents learn more about managing symptoms
through medication and therapy, improving daily living skills,
pursuing educational, vocational and employment goals, solving
transportation needs, addressing substance abuse issues and
increasing their comfort with broader social interaction. Staff
meet with recipients from one to seven days each week to provide
direct services and supportive counseling.
Maple Street is a single-site apartment building with
nine units and 24-hour staffing. Residents maintain separate
addresses and telephone numbers and take responsibility for
the upkeep of their living space.
~ Satellite Apartments, the most independent living level,
uses eleven individual apartments leased by AMH and located
throughout our community. Staff provide services through regular
visits; an on-call system is used in case of emergency.
Supported
Housing ~ Supported Housing helps those ready to leave
psychiatric centers or community residence programs, as well
as homeless people with psychiatric disorders, in finding quality,
permanent and independent housing. A person is assisted in locating
and moving into a new home: finding a location, evaluating a
lease, selecting furniture. After having settled into a new
home, clients regularly work with the AMH Housing Coordinator
to maintain stable living in the community.
